OPTIMAL HEALTH CARE IS CURRENTLY LOOKING FOR REGISTERED NURSES in TALBOT AND CAROLINE COUNTIES Our Home Health Registered Nurses provide services including initial evaluation visits and plans of care.
You will provide high quality care to your patients in the comfort of their own home.
As a Home Health RN, you play a critical role in helping patients progress towards their goals for improved health and independence, and improving the overall health of your own community.
POSITION
Summary:
Provides services in accordance with the plan of care.
Makes the initial evaluation visit (OASIS) and regularly reevaluates the patient's nursing needs.
Initiates the plan of care and necessary revisions.
Provides those services requiring Registered Nurse skills.
Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
Appropriately utilizes ICD-10 codes.

POSITION REQUIREMENTS:
All Candidates for Home Health RN must have:
Graduated from an approved school of professional nursing and currently licensed in the state(s) in which practicing.
Two (2) years' nursing experience, preferred.
